Elderly care at Heart of England NHS Foundation Trust
The elderly care service is a multi-disciplinary team that serves the local populations of Eastern and Northern Birmingham, Solihull and neighbouring communities.
The team provide local services on each of the Solihull, Heartlands and Good Hope Hospital sites with elderly care consultants based primarily around the services centred on one or other of the hospital sites.
The elderly care service has a progressive outlook that is guided by the experiences of older patients and their carers, has a strong emphasis on the education and development of staff.
It is keen to promote new and more effective methods of care in partnership with colleagues in community health and social services. Four of the existing elderly care consultants at Heartlands, two at Solihull and three at Good Hope have joint commitments to community services.
The directorate is continuing to modernise and integrate its services with community teams with the current emphasis being on elderly care consultant support to A&E, safe admission avoidance, supported discharge, integrated old age psychiatry service and a hospital wide focus on dementia / delirium.
Recent developments across the three sites in the last five years include:
Short stay elderly care units on all three sites
Purpose built local intermediate care services
Acute stroke service including thrombolysis across the three sites
Seven day access to TIA services
Integrated falls service and tilt table assessments
Integrated T&O hip fracture service and follow on falls/osteoporosis clinics
Nursing home support service
Rapid access day hospital frail elderly assessment clinics
‘Front door’ elderly care consultants in A&E at Heartlands and Good Hope
Multi-disciplinary Parkinson’s disease service and community ‘Partners in Parkinson’s’ educational programme
Old age psychiatry service at HEFT including Heartlands Elderly Care Day Hospital joint memory clinics as part of a commissioner led implementation of a city wide psychiatry RAID (rapid assessment interface and discharge) service.

Support patients with dementia by knitting hand muffs
Heart of England Foundation Trust is looking to capture the artistic talents of local people to create Twiddlemuffs for dementia patients.
If you are not familiar with Twiddlemuffs, they are hand knitted muffs, which have been specially designed with the addition of buttons, beads, ribbons, zips, designed to provide simple stimulation for active hands.
