Freedom of Information Request: 0139 2019/20

I would like to request a list of surgical errors or ‘never events’ for Heartlands Hospital for the periods listed below:

April 2017 to March 2018 

April 2018 to March 2019

April 2019 to date.

Where possible, the name of the surgical team/consultant should be included.

  • June 2017 – Retained foreign object (instrument insulation) during laparoscopic cholecystectomy Team: General Surgery
  • October 2017 – Wrong side ENT procedure (attempted removal of foreign body) Team: Ear Nose Throat

April 2018 to March 2019
