FOI 0140 2019/20 Surgeons Notes

Freedom of Information Request: 0140 2019/20

I would like to request the surgical notes for Mr Chandock’s surgery for Monday 13 May 2019. This information should include the preliminary schedule, any revised schedules along with clarifying notes throughout the day, and the justifications associated to any changes that were made.

I accept that the names of patients will need to be redacted. Otherwise all other information should be made available.

The Trust does hold the information you have requested. After review we have made the decision to withhold a large amount of the recorded information under Section 40 (2) (This exemption covers the personal data of third parties) of the Freedom of Information Act. This is because disclosure would breach the Data Protection legislation as the date relates to individual data subjects and disclosure would contravene GDPR principle (a) because there is no lawful basis for the processing.

 

Once redacted there was minimal information remaining on the document and this provided no context, therefore we have made the decision not to disclose anything.

We can however inform you that on the 13th May 2019 there were 5 patients scheduled for a procedure under the named consultant. These 5 procedures were performed, and there was no change to the preliminary schedule.  The schedules are determined by the operating consultants and may differ to the schedules recorded pre-operatively within our recording system.

 

Due to the way the NHS works surgical schedules are often revised, or planned procedures postponed, if surgeons are required for unforeseen emergency procedures.
