FOI 0156 2019/20 End-Of-Life Careplan

Freedom of Information Request: 0150 2019/20

  1. The percentage of patients who have died on the Trust’s end-of-life care plan against the Trust’s target for the number of patients who should die on this plan over the last three years, divided by the year. e.g. Between 1 January 2016 and 31 December 2016, 43 per cent of patients died on the plan against a target of 35 per cent.

 

  • The Trust did not have a formal End of Life Care plan during this time.

 2. The amount received by the Trust, via the Commissioning for Quality and Innovation (CQUIN) system or similar, for hitting targets related to its end-of-life care plan for the last three years, divided by the year. e.g. Between 1 January 2016 and 31 December 2016, the Trust received £X via CQUIN for achieving goals involving its ICP.

We have had no CQUIN’s relating to End of Life Care planning; therefore we have not received any income for this.
