FOI 0210 2019/20: A&E Patient Safety Checklist

University Hospitals Birmingham NHS Foundation Trust (UHB) completed a merger by acquisition of Heart of England NHS Foundation Trust (HEFT) on 1st April 2018.  Due to historical differences in data collection/reporting across UHB and the former Heart of England NHS Foundation Trust this response has been provided by hospital site.

Queen Elizabeth Hospital Birmingham

  1. Does your emergency department use a patient safety checklist? If yes how long has this been in place roughly? IF NO, then no further questions need be answered – thank you!  Yes, the emergency department has a patient safety checklist. This has been in use for approximately 18 months.
  2. Is this checklist completed electronically or on paper? The patient safety list is completed on paper
  3. Is the checklist completed in all areas of the ED? ie. resus, majors, minors, paeds rapid assessment etc. If not which areas is it used in? The ED checklists completed are Resus and majors
  4. Please attach the patient safety checklist used Please see attachments 1 & 2 Attachment 1 Attachment 2
  5. How is the compliance of the patient safety checklist monitored in your Trust? The compliance has not been routinely monitored; however we have recently implemented internal audits which look at documentation completion, including the safety checklist. The audit documentation is also attached.
  6. What was the compliance for April? We do not this information

  1. Does your emergency department use a patient safety checklist? If yes how long has this been in place roughly? IF NO, then no further questions need be answered – thank you! Yes, the Emergency Department does have a patient safety checklist and it has been in use for approximately 2 years.
  2. Is this checklist completed electronically or on paper? The patient safety checklist is completed on paper
