Freedom of Information Request: 0249 2019/20

1. How many (a) medical staff and (b) non-medical staff were disciplined in the last two calendar years (2017 and 2018) for improper/illegal or inappropriate use of patient records?

a) UHB has had a total of 9 clinical staff disciplined in 2017 and 2018 for inappropriate use of patient records
b) UHB has had a total of 8 non-clinical staff disciplined in 2017 and 2018 for inappropriate use of patient records

2. Please provide for the five most recent incidents a brief description, which should include

(i) the nature of the offence or breach of Trust policy (e.g. looked at health records of neighbour/celebrity/family member without professional purpose) 2 x looked at own records, 1 x looked at patient records, 1 x discussed wrong patient records, 1 x looked relatives records

(ii) the person’s job title (e.g. nurse, admin staff, physio, consultant)  The trust does hold the information you have requested. However, we believe this information is exempt under Section 40 (2) of the Freedom of Information Act. This exemption covers the personal data of third parties.

(iii) how the matter was resolved, i.e. written warning, sacked etc. 2 x dismissed, 1 x informal action, 1 x final written warning, 1 x case still ongoing.
