Re. Reasonable adjustments:

 

  1. Do you have a ‘flagging’ system in place to notify hospital staff when a patient has a learning disability?

                                          i.    If so, what is it?

Currently the Trust does not have a ‘flagging ‘ system. A scoping work has been undertaken and discussions held with IT team to inform the next steps.

 

  1. How many patient information leaflets do you have currently available across your trust, and how many of these are available in easy read format?

1114 patient information leaflets are available via the Patient Advice and Information. Database (PAID). This is an electronic means of controlling and distributing patient information leaflets through the Trust’s Intranet.  Forming part of the Trust’s electronic patient record development (EPR) the system is set up to be patient centred; a leaflet can only be printed when the system has a specific patient selected.  Due to the interaction of the PAID system with the EPR each time an information leaflet is printed a record is automatically entered into the electronic patient record for that patient, allowing staff to view a complete history of patient information leaflets issued to any patient.

The Health Information Team will ensure information is written in plain language. A “large print” version of the information leaflet is always available if required.

 

 

  1. Do you routinely offer personalised easy read format appointment letters/other correspondence to patients with a learning disability?

No, however the maternity service have developed easy read format appointment letters and are implementing it.

 

  1. Do you routinely offer the option of longer out-patient appointments to patients with a learning disability?

 

  1. Do you routinely offer ‘hospital passports’ to patients with a learning disability during pre-admission appointments?

                                          i.    If so, are your ward staff trained to use them?

Hospital Assessment – Traffic Light system and My Hospital Book resources are

available on Equality & Diversity webpage for staff to use for their identified patients

with a learning disability. Staff training provides participants with complete      information on how to use these documents.

 

  1. What is your process for recognising support needs in a referral to services in your trust, identifying reasonable adjustments that are known to be helpful to the patient, and putting them in place?

Health Facilitation Nurse teams are involved to support the hospital staff in identifying and implementing reasonable adjustments i.e. using various types of sign language interpreters; side room for privacy and dignity; over night stay for carer; flexible time for consultation/tests etc

 

  1. Do you have a policy or written guidance on reasonable adjustments in place for patients with a learning disability?

Yes the Trust has a

The policy is available on Trust intranet page ‘Policies and Procedures‘ for all Trust staff to access. Information is also shared at staff training sessions.

Trust has honorary contract with two Learning Disability Health Facilitation Nurses from Coventry & Warwickshire Partnership NHS Trust. Two Health Facilitation nurses from

Birmingham Community Healthcare NHS Trust work closely with Trust staff on all aspects of

care for patients with a learning disability.
