How to access Trust information
Information can be accessed in one of two principal ways under the Freedom of Information Act: either by searching our publication scheme for information we routinely publish or, when the information is not already published, by making a specific written request.
Option 1: By searching our publication scheme
Our publication scheme represents our commitment to proactively publish information that is readily available to the public. It is reviewed regularly for updates and improvements.
Our publication scheme is classed into eight categories listed below. Wherever possible, we have tried to make information available electronically by simply clicking on the description of the information detailed under each category.
Who we are and what we do
The services we offer
What we spend and how we spend it
What are our priorities and how we are doing
How we make decisions
Our policies and procedures
All Trust policies and procedures are available upon request. A published selection can be found here.
Lists and registers
Information available upon request:
o Gifts and hospitality register (Board members and senior staff)
o Information asset register
o List of main suppliers
o Register of interests (Board members and senior staff)
There are currently no Trust datasets required to be published under the requirements of the Freedom of Information Act
All information detailed in our publication scheme is available in electronic format free of charge. Most information specified is also available in hard copy free of charge upon request.
Option 2: By making a specific written request
If you cannot find the information you are looking for or it is not currently available electronically in our publication scheme, please contact the Trust FOI Office to request this information via one of the following methods:
By email: email@example.com
The Publication Scheme Coordinator Directorate of Safety and Governance,
Devon House, Heart of England NHS Foundation Trust,
Bordesley Green East,
Valid requests under the act must be made in writing and contain an email or postal address for reply.
You will get the best result if your request is as clear and specific as possible about the information you want rather than asking general questions. It is important to note that the focus of the act is about requesting recorded information that the Trust may hold.
Before making a request, you might find it useful to consult guidance issued by the Information Commissioner’s Office on how to make a valid request. If you still require advice, or would like help with formulating a request, please do not hesitate to contact the Trust FOI Office on 0121 424 2549.
How to provide feedback on our FOI services
It is important to us that our FOI services (publication scheme and how we handle requests) meets your needs. If you have any feedback (good or bad) or suggestions on how we can improve our services, please contact the FOI Office as above or complete an FOI feedback form.
