From Friday 1 September, some of the Solihull community services for 0 – 19-year-olds that are currently provided by Heart of England NHS Foundation Trust (HEFT) will be transferring to South Warwickshire NHS Foundation Trust (SWFT) on 1st September 2017.
These services are:
- Health Visiting
- Family Nurse Partnership
- Infant Feeding
- Generic School Nursing services
What does this mean for you?
If you or your child is currently receiving care from HEFT, your care package will continue with SWFT as of 1st September 2017 which will mean that SWFT will require access to these patient records. This will include access to the electronic patient records as well as any paper record currently held by the services at HEFT. This process will ensure that all relevant information can be accessible to promote your health and wellbeing and ensure continuity of care.
Please be aware all health professionals have a duty of care to share information whenever there are safeguarding concerns.
