I would be grateful if you could provide me with a copy of the following:

  • Terms of reference for the Thrombosis committee
  • Reporting arrangements for the Thrombosis committee
  • Agenda a minutes of the last meeting

Further information:

At the time of the request, there was one meeting for the three sites (Heartlands, Good Hope, and Solihull Hospitals) led by the HGS Thrombosis Lead at the time.  Performance was reported separately at the time however this was fed into monthly Divisional Speciality Performance and Quality Meetings split by QE and HGS until the data was integrated.

As of October 2019 a Thrombosis Lead for UHB Trust has been approved and it will be this person’s role to bring together one integrated team for UHB, please note processes are still being aligned.
