Please provide the following information about the disciplinary cases under MHPS in your organization for the financial year 2017/18:
- The number of permanent and locum doctors employed by the Trust (Headcount) 2434
- The number of concerns raised under MHPS by type of concern (the following is a suggested categorisation of the types of concern). Please allocate the cases to the most relevant type of concern.

The Trust does hold information on the number of concerns raised under MHPS by type of concern, but we are withholding this information under Section 40 of the Freedom of Information Act as the data is limited, and it may be possible to identify a doctor.
