The responses to your query are detailed below:

 

(1) How many  CHC checklist forms were completed electronically during 2013  from Good Hope Hospital?  1730

 

(2) How long are the checklists usually kept on the system? Indefinitely although only available by request to IT team once patient is discharged

 

(3) Is the process entirely electronic or are paper copies kept? Entirely electronic since 2012 – paper copies kept prior to this

 

(4) How many checklists were lost accidentally from the system during 2013?   None

 

(5) How many checklists were deleted within a week of submission? 148

 

(6) How many checklists were deleted from the system within 30 minutes of submission?  None
