- We have been unable to answer the questions (2-4) on doctors hours as this would exceed the staff time limit of 18 hours.
- We also have not been able to provide never event information (question 5) further back than 2009. The Never Event definition came about in 2009 and we have included figures since then. Our Datix archive covers this period but to extract what are now considered Never Events would involve apply the criteria from the reference you mention and searching approximately 60,000 records in general then a detailed search of those which meet the broad criteria. This would also exceed the staff time limit of 18 hours.
