3. Can you provide an organisation chart for IT Department including names and job roles?

4. How much money was spent on IT in UK £ excluding trust staff costs in 2014, 2015, 2016?

5. Has the trust had any serious IT outages in the last 3 years and what were they caused by?

6. Can you provide a copy of the current IT Strategy Document?
