Does the current system fulfil the requirements of the policy?
Yes the tools within the policy do support staff in the decision making process. There are a number of quick reference guides for staff to access within the policy to guide them through the decision making process.
Do you currently outsource any 1-1 care for patients displaying challenging behaviours or have to rely on backfilling or security?
We utilise a number of options as per policy either the enhanced care can be accommodated from within the current ward/ department staffing levels on the shift The policy defines the level of observation required in quick reference guide number 1 on page 6 If possible and appropriate we may be able to cohort patients into an enhanced observation bay but this is dependent on patient’s needs and obviously the patients gender.
We can utilise our internal clinical bank for additional Health Care Assistant (HCA) or Registered Nurse (RN). If a Registered Mental Health Nurse RMN is required there are number who hold a temporary clinical bank contract, if we cannot fulfil the shift through this route we will go out to an external agency for a RMN
If out-sourced which department or individuals would be responsible for deciding to call additional staff onto site?
The first assessment is completed by the ward staff this is completed using The Acuity tool (Quick reference guide number 3) in the policy see page 11

How often does the Trust require assistance with patients displaying challenging behaviours?
Due to the complexity and size of the organisation and the Trust’s priority on Patient Safety we have patients with enhanced observations on a daily basis; this will change shift by shift and day by day. The Acuity tool (Quick Reference guide number 3) identifies whether the one is required for either day or night time as it can vary depending upon the patient needs.
