I would like to request retrospective anonymised data for the adult Audiology dept under the Freedom of Information Act for the past five years.
The data which I require to analyse will include:
 Number of referrals made to audiology
 Number of appointments attended and not attended (if possible to include initial appointments, hearing aid fitting appointments and follow up appointments)
 Ethnicity
 Gender
 Age
I would like the data for the previous five years.
