I’d like to know the following please, under the freedom of information act, regarding patient records and data.

1. Does your organisation use handwritten notes for patients?

Yes

2. How is the data stored on your system e.g. excel spreadsheets, word?

In patient paper record and electronic clinical systems such as iCARE and Concerto.

3. How many times have patient records been recorded as lost or stolen in the last financial year?

There have been no records stolen or lost in the last financial year .There were 3179 records unavailable for out patient clinic appointments out of 1,147,824 outpatient appointments.
