University Hospitals Birmingham NHS Foundation Trust (UHB) completed a merger by acquisition of Heart of England NHS Foundation Trust (HEFT) on 1st April 2018.  For the purposes of this request we have answered on behalf of the pre-merger UHB i.e. Queen Elizabeth Hospital.

We wish to request copies of all protocols the Trust holds in relation to antibiotic prophylaxis, that were in place at Queen Elizabeth Hospital Birmingham for the year of 2015, along with all update versions since, to date.

We also request any and all information related to the above.
