Self-administration of medicines policies

  1. Does your Trust have a self-administration of medicines policy for competent patients?Yes
  2. If yes, please provide a copy of this policy? (see attached – if any of the links are required please advise) Patient Self-Administration of Medicines in Adults Procedure
  3. If yes, please outline whether this policy applies to all hospital sites and all wards, or specify the sites or wards to which it applies? All sites and wards
  4. If no, are you planning to introduce this policy in 2017/18? N/A

 

  1. If you have a policy please provide details of the use of the self-administration of medicines policy? This includes:
  2. How are your staff informed about this policy? On all staff intranet and staff are informed in the regular communications bulletin.
  3. How is the implementation of this policy monitored across the hospitals in your Trust? The criteria for monitoring the procedure are included in the procedure at page 12

Carers visiting hours

  1. Do you currently have a policy which allows carers to visit the person they care for outside of visiting hours? We do not have a specific policy but the trust introduced open visiting on 1 April 2015 and the details of this can be found on our website
  2. If yes, please provide a copy of this policy
  3. If yes, please outline whether this policy applies to all hospital sites and all wards, or specify the sites or wards to which it applies
  4. If no, are you planning to introduce a policy in 2017/18? N/A
  5. If no, please outline any work that your Trust is undertaking with a view to enabling carers to visit the person they care for outside visiting hours? N/A
